In-Home Services
Purpose: To provide practical and supportive services to individuals who need help with essential daily activities in the areas of home management and personal care.
Services include:
Social Work Services:
- Provides an assessment of need
- Knowledge and access to community resources
- Development of a service plan and monitoring of service delivery
- Education, resource referral and advocacy for services and resources
Community Social Service Technicians:
- Assist with home management tasks such as changing of bed linens, mopping and vacuuming, laundry assistance, and shopping. Assist with personal care tasks such as dressing, bathing, ambulation, and meal preparation.
