Description

Henderson County Department of Public Health is accepting applications for a full-time Public Health Nurse II- Care Manager position in the Community Health Nursing Unit’s Care Management for High-Risk Pregnancies (CMHRP) program. The primary purpose of the Community Health Nursing unit is to prevent adverse pregnancy outcomes, improve birth outcomes, promote use of the medical home, link children and families to community resources and provide education and family support for ongoing health care to ensure the highest quality of care. Services are provided in diverse settings including the health department, patient homes, childcare facilities, schools, and the community.  

 

Schedule

The schedule is typically Monday through Friday, 8:00 AM to 4:30 PM, with some extended hours (including weekends) to meet client needs and during public health emergencies.  

 

Qualifications

Considerable knowledge of and skill in the application of nursing theory, practices, principles and techniques employed in the field of public health and related programs; considerable knowledge of and the ability to apply principles and practices of public health; knowledge of available resources and organizations and the ability to coordinate these as needed; general knowledge of current social and economic problems relating to public health, including health disparities; ability to plan, coordinate and oversee work of others; ability to deal tactfully with others and to exercise good judgement in appraising situations and making decisions; ability to work in partnership with patients and with other service providers to elicit needed information and to maintain effective working relationships; ability to record accurately services rendered and to interpret and explain records, reports and medical instructions; adequate computer skills to allow communication, patient record documentation, and accessing of information. 

Preferred Qualifications

Previous care management experience. Previous experience in pediatric or obstetrical nursing. Spanish language skills.   

Required Minimum Training and Experience

A current N.C. Registered Nurse license, graduation from an accredited school of professional nursing and one year of professional nursing experience.

Minimum Annual Salary: $70,019.87

Grade:     79A

Hiring salary will be determined based on qualifications and internal equity.
